What to Bring to Your First Visit
- Identification
- Your insurance card(s)
- Any referrals (if required by your insurance company)
- Any x-rays that our physicians have ordered for you
- A list of medications that you are currently taking
- Any co-op payments that are required at the time of the visit (if applicable)

Procedure Instructions for Patients
If you are scheduled for one of the following procedures, you will find preparation and follow-up procedures listed on these pages:
- Intravenous Pyelogram (IVP)
- Prostatic Ultrasound and Biopsy
- Brachytherapy or Radioactive Seed Implantation
Urology Associates of Wisconsin, S.C., is a provider for Medicare and most major insurance plans. We provide insurance billing.
We request payment at the time of your appointment for services that are not covered by insurance. We accept MasterCard and Visa. When necessary, our staff will work closely with patients who require a payment plan.
If you have any questions regarding which insurance plans we accept or any patient billing concerns, please contact us. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
